  • Photo & Signature specifications:
    • Size: 10 KB – 20 KB
    • Photo: 145px × 168px
    • Signature: 168px × 57px
  • Fill all mandatory fields before submitting the application.
  • The application form must be completed and submitted online with all required supporting documents.
  • After submission, take a printout of the Application Form. The ACKNOWLEDGEMENT NUMBER is important for future communication.
